You should receive a confirmation email within 30 minutes of scheduling.  If you do not please give us a call at (480) 991-2820.  Please note if you do not receive an email you are NOT scheduled.

To use the scheduling system you just need your user id and password from last year.  If you have forgotten your password the scheduling system can email one to you. 

In you need a home appointment you need to know your Zone.  Just click on the zone link below and then enter your zip code. 

Zones for home appointments only (not needed for office appointments)

Once you have made your scheduler selections just advance through the calendar, 2 hour open appointments are in white.  Click any open time that is convenient for you.
